If you were able to qualify for a medical wig or post-mastectomy garments and prosthesis through your insurance, but you have high cost shares and deductibles, we may be able to help. Our Financial Assistance Program helps patients afford the out-of-pocket costs for these medically necessary items.

If you are uninsured or unable to qualify for a medical wig or post-mastectomy garments and prosthesis through your insurance, you may request to receive a free wig. These wigs may be new or gently used and are generously donated to us by other cancer survivors, beauty salons, and wig manufacturers.

We have partnered with Otofonix to provide hearing aids to enables provide free hearing aids to individuals facing mild to moderate hearing loss. This program is specifically designed for those who are uninsured or facing financial constraints that make it difficult to afford hearing aids on their own.

How to Apply
STEP 1:
Check your eligibility
Read the Eligibility Criteria section and the federal poverty income guidelines below to determine if you are eligible for financial assistance. You must meet all criteria outlined in this section to apply for financial assistance.
STEP 2:
Gather your documents
For faster verification and processing, please have your the following documents handy: a picture ID, a prescription from your doctor, and a proof of income or letter of support from your family member or friend.
STEP 3:
Fill out the form
If you meet the criteria outlined above and have all the supporting documents, please fill out the application below. One of our Patient Care Specialists will reach out to you within 24-48 business hours to discuss the next steps.

"You Shop, We Give" Program
For every dollar you spend on a qualifying item, we match a dollar and add it to our wig donation fund.
Wig Giveaway Program
With the help of our donors and supporters, we host a wig giveaway where we donate a $600 value medical wig to a cancer patient.
Wig Exchange Program
You're able to donate your gently used or new wig, and we find a cancer or alopecia patient in need who can put your gift to good use.

Our efforts are completely free of charge to you. When you apply for a wig through insurance with us, we reach out to your insurance company to get a coverage amount for you. We provide you with an online catalog of wigs to choose from based on the amount your insurance plan covers. Once you have selected a wig you like, we ship it out to your door at absolutely no cost! After your wig is delivered to you, we work with your insurance company to file a claim on your behalf and take care of any other necessary paperwork.
